Have suffered an illness or injury resulting in a physical disability, which substantially limits daily activity (e.g. paralysis, blindness, cancer, amputation, or head injury). Meet the admission requirements of a four-year or graduate-level institution. Make satisfactory progress towards your degree after college or graduate school acceptance. Maintain a minimum of 2.5 GPA cumulative and be "in good academic standing" at your accredited school. Demonstrate financial need.

The John Lepping Memorial Scholarship has been designed by the Lep Foundation for Youth Education (a public charity that provides financial assistance to students residing in NY, N J or PA seeking to further their education). Including, but not limited to physical conditions (spinal cord injury, loss of limb, birth defects, etc.) and psychological conditions (autism, post- traumatic stress etc.). Scholarship recipients must be able to provide proof of disability with letter of confirmation by a physician. Proof required after award notification. Do not submit with application. The recipient of the Dakota Pequeno Memorial Scholarship will be a student who has shown perseverance in eliminating obstacles that epilepsy and seizures may present, courage in dealing with epilepsy/seizures, community involvement, and commitment to their education. Eligibility Requirements: Must have a diagnosis of epilepsy/seizure disorder and confirmed by a letter from a physician. Must be a high school senior or a high school graduate between the ages of 17-25. Must live in Michigan. For applicants going to school out-of-state, your permanent address must be located in Michigan. Must provide proof of acceptance to a post-secondary academic or vocational program. Must submit a completed application and one letter of recommendation, preferably from an educator. Write a short essay regarding epilepsy.

Created in memory of a young athlete who committed suicide in 2006 after losing his battle with mental illness, the Drake Davis Foundation Scholarship is granted annually for graduating high school seniors who have overcome a mental health diagnosis and are dedicated to helping with the prevention of suicide. Qualified candidates must be accepted at an accredited U.S. higher learning institution, exemplify outstanding character, show leadership potential, and submit one letter of recommendation from a school official.

The following criteria are used for selection of scholarship recipients to ensure that the mission of the Foundation is supported. %u200B Recipients must be a citizen or permanent resident of the United States. Recipients must have significant bilateral hearing loss. We require at least a 50 dB unaided hearing loss in both ears (or cochlear implant(s)), and we accept applicants who have a cochlear implant on an equal basis as those without an implant. Recipients must be undergraduates attending an accredited non-profit college physically located in the United States. The Foundation will not consider applications for graduate or high school tuition. %u200B Applicants who meet the above criterion are considered regardless of age, sex, race, geographic location or ethnicity. The final selection of scholarship recipients will be made by the Foundation's board of directors or by designees as directed by the board.
